Fixed in v1.6.9 (686b80b).

Root cause was the run counter, not the rate: any backwards move opened a new pipeline run, and stage detection bounces within a single feature (implement → tests → implement). On this lane that recorded 26 runs where 5 features had shipped. A backwards move now opens a run only when it is declared; a backwards detection continues the run in flight.

Note the numbers stay low for work done in plain chat: a pass is a declaration (agenthub stage), and a session that never calls it leaves detections (the grey bar), not passes.
