Đã sửa trong v2.5.11 (commit 6ef9a5d4).

Nguyên nhân: panel Changes đọc trạng thái bằng git status --untracked-files=normal. Ở chế độ này git gộp cả một thư mục chưa được theo dõi thành một dòng duy nhất (docs/, .phpunit.cache/), nên panel không mở, không stage và không discard được file nào bên trong.

Cách sửa: chuyển sang --untracked-files=all (cách VS Code và JetBrains đang dùng). Giờ mỗi file mới hiện thành một dòng riêng, ví dụ docs/sonar-high-issues.md và .phpunit.cache/test-results. Từng file mở được, stage hay discard riêng được; số đếm trên header cũng khớp với danh sách.

Đã có test tái hiện đúng trường hợp này (thư mục mới chứa nhiều file). Anh/chị cập nhật lên v2.5.11 bằng /agenthub:agenthub-update là thấy.
