nntrivi2001 b7c573f8d9 feat(plugin): ship the repo's skills as ccam plugin skills
The repo root already IS the `ccam` plugin root (`"source": "./"`), so the
conventional `skills/` dir at the root is the plugin's skills dir — while
`.claude/skills/` only loaded for a session whose cwd was inside this
checkout. That is useless for a lane working in another repo, which is why
`ccam skills install` existed at all.

Move all seven skills to `skills/`: they now reach every plugin user as
`/ccam:<name>` (bare `/<name>` as an alias). `ccam skills install` stays as
the fallback for a plain checkout without the plugin, sourcing from the new
path.

Agents deliberately do NOT move. A plugin agent is addressed as
`ccam:qc-local`, which would break the `subagent_type: qc-local` references
in ship-feature-lane; `ccam lanes agents install` keeps writing them into the
lane's own `.claude/agents/`, the highest-priority agent source. MCP was
already plugin-shipped via `plugins/ccam/.mcp.json`.

Mechanical fallout: ~115 source files carry a doc-comment pointer to
`.claude/skills/file-headers/`, repointed to `skills/file-headers/`. The
generic Claude Code config-surface examples in openapi/cc-config (`.claude/
skills/code-reviewer`) describe any user's layout and are left alone.

Also fixes the two file-headers scripts, whose `ROOT=` climbed four levels
from `.claude/skills/file-headers/scripts/` and would have escaped the repo
from the new depth — it scanned sibling projects until corrected to three.

npm run test:server (1160 pass), test:client (422 pass), mcp:typecheck,
mcp:build + mcp:check-build all green.

/**
* @file server.ts
* @description Main entry point for building the MCP server. This module defines the buildServer function, which initializes a new MCP server instance with the provided configuration, API client, and logger. It also registers all tools by calling the registerAllTools function, which sets up the tool handlers for the server. The buildServer function returns the configured MCP server instance, ready to be started and handle incoming requests from the MCP client. This module serves as the central place for assembling the server components and ensuring that all necessary tools are registered before the server starts.

* **Purpose:** Dashboard module consumed by the React client, MCP tools, or desktop shell depending on deployment mode.
*
* ## Design constraints
* - Local-first: no telemetry leaves the machine unless the user configures webhooks.
* - Fail-safe hooks path on the server must never block Claude Code; UI mirrors that
* philosophy by degrading gracefully (empty states, stale badges, reconnect loops).
* - Destructive flows stay behind explicit confirmation modals and server-side gates.
* - Internationalization: user-visible strings belong in i18n JSON, not literals here.
*
* ## Remote data & SSH
* Remote Data Sources let operators aggregate multiple machines. SSH entries describe
* how to reach a peer dashboard; the global data scope (`dataScope.ts`) narrows every
* scoped GET via `?sources=`. Health checks and import history surface in Settings.
*
* ## Internal dependencies
* - `./config/app-config.js`
* - `./clients/dashboard-api-client.js`
* - `./core/logger.js`
* - `./tools/index.js`
*
* ## Public surface
* - `buildServer` — exported API; see TSDoc on the symbol for behavior.
*
* ## Testing pointers
* - Prefer colocated `__tests__` with Vitest + Testing Library for UI.
* - Server contract changes require `npm run test:server` and OpenAPI sync.
* - MCP edits: `npm run mcp:typecheck` and `npm run mcp:build`.
*
* ## Related docs
* - `ARCHITECTURE.md` — hooks → API → SQLite → WebSocket → UI pipeline.
* - `docs/API.md` — REST reference.
* - `skills/file-headers/` — mandatory `@author` header policy.
