nntrivi2001 b7c573f8d9 feat(plugin): ship the repo's skills as ccam plugin skills
The repo root already IS the `ccam` plugin root (`"source": "./"`), so the
conventional `skills/` dir at the root is the plugin's skills dir — while
`.claude/skills/` only loaded for a session whose cwd was inside this
checkout. That is useless for a lane working in another repo, which is why
`ccam skills install` existed at all.

Move all seven skills to `skills/`: they now reach every plugin user as
`/ccam:<name>` (bare `/<name>` as an alias). `ccam skills install` stays as
the fallback for a plain checkout without the plugin, sourcing from the new
path.

Agents deliberately do NOT move. A plugin agent is addressed as
`ccam:qc-local`, which would break the `subagent_type: qc-local` references
in ship-feature-lane; `ccam lanes agents install` keeps writing them into the
lane's own `.claude/agents/`, the highest-priority agent source. MCP was
already plugin-shipped via `plugins/ccam/.mcp.json`.

Mechanical fallout: ~115 source files carry a doc-comment pointer to
`.claude/skills/file-headers/`, repointed to `skills/file-headers/`. The
generic Claude Code config-surface examples in openapi/cc-config (`.claude/
skills/code-reviewer`) describe any user's layout and are left alone.

Also fixes the two file-headers scripts, whose `ROOT=` climbed four levels
from `.claude/skills/file-headers/scripts/` and would have escaped the repo
from the new depth — it scanned sibling projects until corrected to three.

npm run test:server (1160 pass), test:client (422 pass), mcp:typecheck,
mcp:build + mcp:check-build all green.

/**
* @file EmptyState.tsx
* @description Centered empty-state panel used whenever a page or section has
* nothing to render yet — no sessions, no events, no search hits, or a feature
* that has not been configured. Keeps the UI from looking broken by giving the
* user a clear icon, title, explanation, and an optional call-to-action slot.
*
* ## When to use
* Prefer this over ad-hoc "No data" paragraphs so every list/table page shares
* the same vertical rhythm, typography, and card chrome. The optional `action`
* slot accepts any React node (usually a `<Link>` or `<button>`) without this
* component needing to know about routing.
*
* ## Accessibility
* The icon is decorative (no separate `aria-label`); meaning comes from the
* visible `title` (`<h3>`) and `description` (`<p>`). Callers should pass
* translated strings via `useTranslation`.

* **Purpose:** Dashboard module consumed by the React client, MCP tools, or desktop shell depending on deployment mode.
*
* ## Design constraints
* - Local-first: no telemetry leaves the machine unless the user configures webhooks.
* - Fail-safe hooks path on the server must never block Claude Code; UI mirrors that
* philosophy by degrading gracefully (empty states, stale badges, reconnect loops).
* - Destructive flows stay behind explicit confirmation modals and server-side gates.
* - Internationalization: user-visible strings belong in i18n JSON, not literals here.
*
* ## Remote data & SSH
* Remote Data Sources let operators aggregate multiple machines. SSH entries describe
* how to reach a peer dashboard; the global data scope (`dataScope.ts`) narrows every
* scoped GET via `?sources=`. Health checks and import history surface in Settings.
*
* ## Public surface

*
* ## Testing pointers
* - Prefer colocated `__tests__` with Vitest + Testing Library for UI.
* - Server contract changes require `npm run test:server` and OpenAPI sync.
* - MCP edits: `npm run mcp:typecheck` and `npm run mcp:build`.
